Before You Ever Sit in the Chair

Meet the Practice First

Choosing a new dentist is easier when you know who you're walking in to see. Take a quick look inside the office and get a feel for how Dr. Collins and the team care for patients, then read more about the team behind the practice.

It's the same calm, unhurried approach you'll experience at your first appointment, and every appointment after that.

Before You Arrive

Save Time with Online Forms

Filling out paperwork ahead of time means more time in the chair with Dr. Collins and less time in the waiting room. Download and complete these three forms from our patient resources page, then bring them to your first visit. If you'd rather complete them in office, plan to arrive 15 minutes early.

New Patient Information Form

Basic contact information, insurance details, and emergency contacts.

Health History Form

Medical history, current medications, allergies, and any conditions Dr. Collins should know about.

HIPAA Acknowledgement

Standard privacy form required for all new patients.

Common Questions

New Patient FAQ

Your new patient visit starts with a friendly welcome and a review of your health history. Dr. Collins then performs a complete exam with digital x-rays and a 3D intraoral scan, walks you through every finding in plain language, and builds a care plan around your goals and budget. If time allows, we finish with a cleaning.

Plan for about an hour, sometimes a bit more if we add a cleaning the same day. Completing your forms at home ahead of time keeps the visit moving and gives you more time with Dr. Collins.

Bring your insurance card, a list of current medications, and your completed forms. If you have recent x-rays from a previous dentist, ask that office to send them to our Springfield office before your appointment so we can avoid retakes.

Not at all. Whether it's been six months or six years, you'll get the same warm welcome. There's no lecture and no judgment, just a clear picture of where your oral health stands and a plan to move it forward at a pace that works for you.
